AMIODARONE

Class
  Anti-arrythmic
Action
  Prolongs QT interval.
Indications
 
  • Recurrent Ventricular Fibrillation.
  • Recurrent Hemodynamically unstable ventricular tachycardia.
Contraindications
 
  • Hypersensitivity.
  • Sinus Bradycardia.
  • Sino-Atrial Heart Blocks.
  • Thyroid dysfunction.
  • Pregnancy and breast feeding
Precaution
  • Do not administer with other drugs that prolong QT interval.
  • Terminal elimination is extremely long up to 40 days.
  • With multiple dosing, cumulative doses>2.2g/24 hours are associated with significant hypotension clinical trials.
Side effects
  • Nausea and Vomiting.
  • Hepatotoxicity.
  • Phototoxicity.
  • Thyroid dysfuctions.
Administration
Rout: IV Onset: 1-2 minutes Peak effect: 10 minutes Duration: Variable
Adult Dose:
Stable Wide-QRS Tachycardia
  • First dose 150 mg IV/IO push diluted in 20-30 ml D5W over 10 minutes.
  • Repeat dose 150mgIV if VT recurs
Cardiac Arrest
  • First dose : 300mg IV/IO Push
  • Repeat dose :150mg IV/IO Push
Pediatric Dose:
Stable Wide-QRS Tachycardia
  • First dose: 5mg/kg IV/IO over 20 minutes (Max 150mg)
  • May repeat once if needed
Cardiac Arrest
  • 5mg/kg IV/IO bolus may repeat up to 2 times for refractory VF/Pulseless VT
