ATROPINE SULPHATE

Class
 
  • Parasympatholytic.
Action
 
  • Blocks acetylcholine receptors.
  • Positive chronotrope and increases AV conduction.
Indications
 
  • Symptomatic bradycardia.
  • AV blocks.
  • Asystole.
  • Organophosphate poison.
Contraindications
 
  • Use with caution, if at all, in 2nd degree Type II and 3rd degree blocks.
Precaution
  • Dose of 0.04 mg/kg should not be exceeded except in cases of
  • Organophosphate poisonings.
  • For ET administration, given 2 - 2.5 times the recommended IV dose.
Side effects
  • Palpitations (increased cardiac workload).
  • Pupil dilation.
  • Dry mouth.
Administration
Rout: IV/IM Onset: Immediate Peak effect: 2-4 minutes Duration: 4 hours
Adult Dose:
  • for Bradycardia:1 mg bolus, repeat every 3 - 5 minutes to a maximum of 3mg.
  • Organophosphate poisoning: 2mg IV/IO /IM to start, repeat every 5 minutes as needed until symptoms improve.
Pediatric Dose:
Bradycardia- Only if increased vagal tone
  • 0.02mg/kg IV/IO rapid push Minimum dose 0.1mg and maximum single dose 0.5mg.
  • Repeat Once after 5 minutes if needed
Organophosphate poisoning
  • 0.05-0.1mg IV/IO/IM repeat every 5 minutes as needed until symptoms improve.
