DEFIBRILLATION

  • Defibrillation involves the delivery of non-synchronized direct electrical current (mono or biphasic) to the myocardium of a patient exhibiting ventricular fibrillation or pulseless ventricular tachycardia.
  • The objective of the defibrillation is to depolarize the entire myocardium, which hopefully will result in allowing a single reliable pacemaker site to assume pacemaker control at a rate capable of producing an adequate cardiac output.
INDICATION OF PROCEDURE:-
  • Patient in ventricular fibrillation.
  • Patient in pulseless ventricular tachycardia.
PROCEDURE:-
  • Undertake universal precautions (BSI).
  • Verify the absence of pulse and respiration.
  • Initiate CPR with C-A-B sequence.
  • Place the patient in a safe environment if initially in contact with some electrically conductive material such as metal or water.
  • Connect cardiac monitor to analyze rhythm monitor pulse oximetry and Blood Pressure.
  • Confirm ventricular fibrillation / pulseless ventricular tachycardia.
  • Apply electrode gel to the paddles, or place defibrillator pads on the patient’s exposed thorax.
  • Turn on and charge the defibrillator to 200 J for the shock.
  • Ensure the electrodes are appropriately placed on the patient’s thorax with the proper pressure.
  • Ensure that no one else in contact with the patient, verbally and visually clear everybody, including yourself, before any defibrillation attempt.
  • Deliver a defibrillatory shock the red buttons as per the defibrillator used.
  • Immediately resume CPR for 2 minutes without checking the pulse.
  • Reconfirm the rhythm on the monitor screen, if patient is still in VF/Pulseless VT, recharge the defibrillator to 200J and shock again.
  • For pediatric patient’s same method apply except paddles are smaller and the energy level start with 2J/kg, 4 J/kg, and then 4J/kg.

Contact medical director as soon as possible.

COMPLICATIONS:-
  • Burns to skin.
  • Deactivation of patient’s implanted pacemaker.
  • Injury to patient, self or bystanders.
PRECAUTIONS:-
  • Patient who is fully digitalized may require less then normal recommended delivery energy.
  • Avoid placing paddles over cardiac pacemaker or nitroglycerin patches.
  • Make sure the patient is not in contact with water or metal while defibrillating.
  • Ensure that no one is touching the patient while defibrillating including you.
