FENTANYL (SUBLIMAZE)

Class
 
  • Synthetic Potent Narcotic( Fentanyl is 100 times more potent than Morphine).
  • Analgesic
Action
 
  • A potent analgesic that also causes some vasodilation, thus reducing myocardial oxygen demand.
Indications
 
  • Moderate to severe pain associated with Medical or Traumatic condition
  • Angina, Myocardial Infarction
  • Congestive Heart Failure
  • Acute Pulmonary Edema
  • Burns
  • Isolated Injuries requiring pain relief.
  • Amputations
Contraindications
 
  • Known allergic to Fentanyl
  • Hemodynamic instability or shock
  • Respiratory depression
  • Undiagnosed abdominal pain
  • Patient on other Psychiatric or Anti-seizure medications
Precaution
  • Toxic effects of mixed drug overdose may emerge with the reversal of the benzodiazepine effects.
  • Have Naloxone readily available in case of respiratory depression to counteract the effects of Fentanyl
  • Vital signs must be monitored frequently
  • Hypotension
  • Patient suspected head Injury
Side effects
  • Respiratory Depression (May occur suddenly, and more commonly in children and the elderly)
  • Apnea
  • Hypotension
  • Altered Level of Consciousness
  • Rashes
  • Nausea, vomiting
  • Muscle stiffness
Administration
Rout: IV/IO Onset: Immediate Peak effect: 3-5 minutes Duration: 30-60 minutes
Adult Dose:
  • 1-2mcg/Kg, Slow IV/IO bolus
    • Adult and children over 12 years of age: (50-100 mcg given IV /I.O or I.M)
    • Elderly (65 years of age &>) dose should be given at 1/2 dose (e.g. 25 mcg I.V/I.O bolus SLOW)
  • The dose can be repeated after 10 minutes and titrated to clinical effect to a maximum cumulative dose of 100mcg.
  • Maximum dose 100 mcg
Pediatric Dose:
  • 1 mcg/kg SLOW IV/IO bolus
  • Dose can be repeated after 10 minutes and titrated to clinical effect to a maximum cumulative dose of 2 mcg/kg
