FLUMAZENIL

Class
 
  • Benzodiazepine receptor antagonist.
Action
 
  • Antagonizes the actions of benzodiazepines on the central nervous system.
Indications
 
  • Reversal of benzodiazepine sedation.
Contraindications
 
  • Hypersensitivity to Flumazenil or to benzodiazepines.
  • Cyclic antidepressant overdose.
  • Known seizure disorder
Precaution
  • Toxic effects of mixed drug overdose (especially cyclic antidepressants) may emerge with the reversal of the benzodiazepine effects.
Side effects
  • Nausea and vomiting, dizziness, agitation.
  • Cardiac dysrhythmias.
  • Injection-site pain, cutaneous vasodilation.
  • Abnormal vision.
  • Seizures.
Administration
Rout: IV/IO Onset: 1-2 minutes Peak effect: 6-10 minutes Duration: 20-50 minutes
Adult Dose:
  • 0.2 mg IV/IO push over 30 seconds.
  • An additional dose of 0.3 mg IV/IO may be given in 30 seconds.
  • A third dose of 0.5 mg at one-minute intervals.
  • Maximum dose of 3 mg.
Pediatric Dose:
  • Infant to children (1 -17 years old)
  • Intitial dose: 0.01mg/kg IV/IO/given over 15 seconds,
  • Repeat dose: 0.01mg/kg IV/IO given over 15 seconds, may repeat 0.01mg/kg after 45 seconds.
  • Maximum total accumulation dose: 0.05mg/kg
