Hypertensive Crisis

EMT STANDING ORDERS
  • Routine Patient Care.
  • Place patient in a comfortable Position.
  • Connect to cardiac monitors, attach pulse oximetry and BP cuff.
  • Obtain 3 lead ECG if available.
  • Establish IV access KVO.
  • Consider expert consultation.
  • Call for AEMT/Paramedic intercept. & Assisst AEMT/Parmedic in patient care.
AEMT/PARAMEDIC STANDING ORDER
  • Acquire and transmit 12-lead ECG.
  • Establish IV/ IO access if not established yet.
  • Direct treatment at the primary problem.
  • All symptomatic patients with hypertension should be transported with their head elevated at 30 degrees.
  • Consider nitroglycerin 0.4 mg SL every 5 minutes, Systolic BP should not drop below 100 mmHg or drop quickly.
  • Consider Furosemide 40-80 mg IV/IO, especially if there are sign and symptoms of pulmonary edema.
  • If chest pain is present, follow chest pain protocol.
