MAGNESIUM SULFATE

Class
 
  • Class V Antidysrhythmic, Electrolyte.
Action
 
  • Replaces and maintains magnesium levels.
  • Interferes with the release of acetylcholine at the myoneural junction.
Indications
 
  • Severe refractory ventricular fibrillation.
  • Seizures due to eclampsia.
  • Torsades de pointes.
  • Severe bronchoconstriction with impending respiratory failure.
  • Hypomagnesemia.
Contraindications
 
  • Patients with heart block or recent myocardial infarction.
Precaution
  • Caution should be used when magnesium is administered to safeguard against clinically significant hypotension or asystole.
Side effects
  • Hypotension.
  • Circulatory collapse.
  • Cardiac and respiratory depression.
Administration
Rout: IV Onset: Immediate Peak effect: Variable Duration: 1 hour
Adult Dose:
Cradiac arrest:
  • 2 g slow IV (diluted in 10 mL D5W) over 5-20 minutes
Torsade de Pointes:
  • 2 g slow IV (diluted in 50-100 mL D5W) over 5-60 minutes
Eclampsia:
  • 4g mgso4 to 20% solution IV over 4 minutes
Pediatric Dose:
Torsades de Pointes:
  • 50 mg/kg IV/IO repeat every 5 minutes to Max 2 gms
