Post Resuscitative Care - Adult
EMT STANDING ORDERS:
- Routine Patient Care.
- Optimize ventilation and oxygenation:
- Administer oxygen as appropriate with a target of achieving 92% - 98% saturation.
- Consider advanced airway LMA when it is indicated.
- Maintain the patient's body temperature.
- Call for AEMT/Paramedic intercept. & Assisst AEMT/Paramedic in patient care.
AEMT / PARAMEDIC STANDING ORDER:
- Optimize ventilation and oxygenation:
- Consider advanced airway when its indicated and use waveform capnography to confirm and monitor the correct placement of the ET tube.
- Titrate ventilation rate to waveform capnography of 35 to 40 mmHg.
- Maintain systolic blood pressure of ≥ 90 mmHg (MAP > 65).
- For Post-Resuscitation Hypotension:
- Administer 0.9% NaCl or Lactate Ringer in 1000 - 2000 mL boluses. Total volume should not exceed 2,000 mL.
- Norepinephrine infusion 0.1 to 0.5 mcg/kg/min IV/IO titrated to effect.
- Epinephrine infusion 2 - 10 mcg/min IV/IO titrated to effect.
- Dopamine infusion 5 -20 mcg/kg/min and titrated to effect.
- For Post-Resuscitation Hypotension:
- Acquire and transmit a 12-lead ECG.
- Check patient follows commands and maintains Targeted Temperature Management TTM from 32 - 36 for 24 hours as indicated.
