  • PH buffer, alkalizing agent, electrolyte supplement
Action
 
  • Increases blood and urinary pH by releasing a bicarbonate ion, which in turn neutralizes hydrogen ion concentrations .
Indications
 
  • Prolonged cardiac arrest.
  • Crush syndrome.
  • Tricyclic antidepressant (TCA) overdose.
Contraindications
 
  • Hypocalcemia
  • Hypokalemia
  • Hypernatremia
  • Severe pulmonary edema
Precaution
  • Sodium Bicarbonate can cause metabolic alkalosis when administered in large quantities.
Side effects
  • Metabolic alkalosis
  • Seizures
  • Electrolyte disturbance.
Administration
Rout: IV Onset: Rapid Peak effect: 1-2 minutes Duration: 8-10 minutes
Adult Dose:
Cardiac arrest:
  • 1 mEq/kg (maximum dose of 50 mEq) IV bolus over 5 minutes
  • May repeat once in 5-10 minutes, tailored to response.
Crush injury/syndrome:
  • 1mEq/kg added to 1L of normal saline, run IV wide open just prior to extrication
  • After extrication, consider additional dose after consulting medical direction.
Pediatric Dose:
  • Same as adult but infuse slowly and only if ventilations are adequate
